New patient checklist:
Call our office at 703-689-2020 with your insurance information ready. If you are unsure if you are in network, please call your insurance company. If you are unable to, send an email with the information below to gcwinfo@glaucomadoctor.com along with your contact information and reason for visit. Expect a phone call from the office to schedule.
Pictures of your insurance card and photo ID, front and back
Full name and date of birth
Referring doctor’s name
Reason for the referral (eg. Glaucoma suspect, evaluation, Laser referral, surgery referral, etc)
Best phone number.
Ask your referring doctor to send over your medical records and any testing to our office.
Bring a list of all medications you are currently taking, including multivitamins and supplements, and please bring all eye medications with you.
For a shorter time in the waiting room, please fill out the new patient paperwork and bring it with you to your appointment beforehand. If you are unable to do so, please plan to arrive 15 mins prior to your scheduled appointment time.
Please keep in mind that due to testing and possible dilation, your appointment may take up to 3 hours. We look forward to seeing you!
What to expect on your first visit:
Medical history examination taken by GCW medical assistants.
Glaucoma testing as necessary, including visual field, OCT and retinal topography.
Intraocular Pressure (IOP) reading.
Dilated fundus exam after with a gonioscopy (a special exam to look at the drainage of the eye)
Detailed discussion with the doctor going over results, past medical records, and a developed treatment plan.
Scheduling a follow up appointment as appropriate to ensure continuity of care.
After reviewing your results, records, and having a detailed discussion with you regarding your care plan, and will let you know how to follow up. Your first appointment will take anywhere from 2-3 hours.

We accept Tricare only as a secondary insurance to Medicare.

Please note, if you have an HMO plan, you will need to check if a referral is required, and obtain one from your PCP prior to your visit.
